Enrollment Process for Center-based Programs


Tours/Parent Presentations

Tours and Parent Presentations are conducted at all locations by appointment; please call (614) 602-6482 to schedule. Family members and professionals are welcome to attend. Prospective students are encouraged to visit after the parents have participated in a tour and/or presentation.

Applications

Applications are available at tours or by request; call (614) 602-6482 to obtain an electronic version or hard copy. Application requirements include the completed application, $50 application fee, and the student’s Individualized Education Plan (IEP). Other helpful documentation, if available, includes the most recent Multi-factored Evaluation (MFE), IEP Progress Report, grade report, standardized testing results, behavioral plans/reports, diagnostic work and/or progress reports, if any, from a physical therapist, occupational therapist, speech therapist, psychiatrist, psychologist, or behavioral specialist. Send all applications along with the IEP and application fee, if required, to the Columbus address, attention: Carol Jackson.

Enrollment

If you plan to use a scholarship to pay for services at HLC, we recommend you begin the application process for the scholarship through the Ohio Department of Education (ODE) at the same time you begin the application process for enrollment with HLC. Once we receive your completed HLC application, we will contact you to set up an interview with you and your child. Start dates are determined by space availability, funding agreement finalization, and the completion of support staff placement, if needed.

Students enrolled by July 15th are guaranteed fall placement. Please call HLC to inquire about availability after July 15th.
